I do the same thing......get that first sentence. Walk around....do something that doesn't require you to use your eyes (watching TV or reading is bad). Just step outside, although it's freezing, and look at Orion for a couple of minutes. Get that sentence anr run with it.

I use stream of consciousness writing when i get stuck, write only on one side of the paper, write down all random thoughts as they come, look up definitions and write those down too, keep going until the brains stops then get out the scissors and edit, rearrange all your scraps and tape them in some sembalnce of order and Viola you have an outline
