this may be a little late in the asking, but what exactly did he mean by change? and why's he even asking these question? all he has to do is call AT&T and ask them to send everything they got

Has anyone seen the previous application form (for comparison) or evidence of the legitimacy of this form (that it is what people are saying it is)? All I've seen is a badly scanned PDF of the document that doesn't actually provide any indication with what it is an application for.

It may be accurate, but I always like to question and compare. I'm genuinely curious.

Then again, if it was totally inaccurate I presume that some sort of PR response would be in place.
